Procedure for Registered Schools: How to Resume Service for the Current School Year
Follow these steps to resume library service:
- Read the instructions for completing the Annual Student Listing form. Your library service may be delayed if you complete the form incorrectly and/or omit required information.
- Complete the Annual Student Listing form.
- Read the information about service options for schools.
- If you select "on demand" service for one or more formats, contact the Youth Services Librarian and let her know your decision. If you select one of the automatic service options, complete the service options form and mail it with your student listing form.
- Mail the completed form to Wolfner Library, PO Box 387, Jefferson City, MO 65102-0387. DO NOT FAX the form to the library. The library must have original signatures on file.

What Happens When the Library Receives the Annual Student Listing Form?
The library staff is required to check the names of the students on the Annual Student Listing form against the library’s patron database. In accordance with federal regulations, all students using Wolfner Library services in a school setting must have a certified individual application on file with the library. The library staff will notify you which, if any, students are not on file. The Application for Free Library Service: Individuals is available on the web site, or the library can mail one to you for each student.
